Company Registration

In order to access the on-line medical screening questionnaire and order anti viral medication for you and your employees, you will need a unique reference number (URN). This number registers your company and allows us to ensure that medication is only being provided to businesses, their employees and their families.

To register your company, please down load a template registration request letter.

Please copy this letter onto your company headed paper, completing the details where requested. You may then email this back to us and we will send you your company’s Unique Reference Number (URN). This URN can be used by you and all of your employees to access the medical screening questionnaire.

The manufacturer, Roche, has been asked by the UK Department of Health not to supply their anti-viral medication to members of the public and, therefore, we require a letter of confirmation that the medication is only to be used in a company scheme.

Alternatively you can post your letter back to us at the following address and we will contact you with your URN. In this case please provide us with your contact details so that we can provide you with your URN as soon as possible:
